Please find above a Downloadable & Printable PDF version of the Proxy Form that was sent with the Official Notice to Members for Annual Meeting (AGM)

If you cannot attend The Village Annual Meeting, please fill in this Proxy Form and return via E-mail or Fax so that your vote is represented at the AGM.

Proxies must be filled in correctly in order to be valid.  Please be certain you either check Bailey Provetto as your designee, or print the name of your preferred designee at the top of the Proxy Form.  This person must be someone who will be attending the meeting to vote on your behalf.  DO NOT put your own name in the top part of the Proxy Form.  This will assign yourself as the designee and therefore negate your Proxy.  DO put your own name at the bottom of the Proxy Form and be sure to sign and date at the bottom.  Proxies count the same as if you were present voting at the meeting.  The attached Proxy Form allows the person you designate at the top of the form to vote on any matter that properly comes before the Members at the meeting.
